Help identifying emblem (Lakewood?)

Is this from the back of a 61 Lakewood? I think that's right. I've seen photos of the letters black and silver online just wasn't sure which is stock and or if its for other models as well.

I didn't want to make it available without knowing the details. Unfortunate crack in the plastic but it seems better than most photos I've been able to find. has original box so it could be slight damaged NOS.

Might be difficult putting a value to it as well since I can't seem to find any for sale or available.

Thank you for your help.

Jeff
